Johnny Ray Baize

Johnny Ray Baize, 55, of Providence, KY. passed away on October 8, 2020 at Baptist Health in Madisonville, KY.   He was born on May 11, 1965 in Henderson, KY to John Baize and Evelyn Young Baize. He was a truck driver at Providence Recycling for several years,  and served in the Army Reserve He was preceded in death by his father and a sister, Theresa Lovell He is survived by his mother Evelyn Tapp of Clay, Ky.  Two children, son Michael (Rachel) of Robards, KY and daughter Vanessa O’Brien (David) of Lisman, KY, fiance of 7 years Allison Calvert, Three sisters Angela Baize of Sebree, Veronica Spinks ,of  Dixon, Ky, Mona Lisa Gamblin ,of  Clay, KY and Brother Loman Baize, Providence, KY, 6 grandchildren and 1 great-grandchild. Service will be held at graveside, Springdale Cemetery, Sebree, Ky on October 16, at 2:00 PM  Bro. Audie Cherry officiating. Full military rites conducted by American Legion Worsham Post 40 of Henderson, KY Visitation will be Friday, October 16, from 11:00 AM till 1:30 PM at Tomblinson Funeral Home in Sebree,KY.
