In some instances, a Kentucky Funeral Planning Declaration may be an appropriate addition to your pre-arrangements. This is a legally binding document that allows you to put assurances in place that your final wishes will be carried out. This can be done by appointing a designee (if your legal next of kin is not who you prefer to carry out your wishes) or by simply stating your wishes on the declaration form FPD-1, 04-17.
